How does it work?
Your primary care doctor will:
- conduct an in-depth interview with you
- examine you
- analyze the results of your diagnostic tests
- assess your current health status
- establish an individual action plan with you
- outline the next steps in the process
- provide you with the plan to follow
- you will also have it in electronic form in your Online Patient Account.
