To become a patient at Villa Med, you need to submit a declaration of choice for a doctor or nurse. You can do this conveniently from home—through the Internet Patient Account (IKP).
Step by Step: How to Submit a Declaration via the Internet Patient Account
- Log in to your IKP — you can use Trusted Profile, e-ID, or a bank account.
- Go to the "My Account" tab, then find the "Your Primary Health Care (POZ)" section.
- In this section, click the "Fill Out Declaration" button.
- Select the facility — Villa Med in Białystok — and choose a doctor or nurse, then confirm the declaration.
This way, your declaration will reach our clinic, and all data will appear in your IKP. Changes should take effect within 7 days, but sometimes it may take longer.
Free Changes – When Are They Possible?
Twice a calendar year, you can change your POZ doctor, nurse, or midwife without giving a reason and without extra charges. Additional changes are possible but incur an administrative fee.
Free changes are also allowed in exceptional situations, such as:
- changing your place of residence,
- clinic closure or doctor retirement,
- you or your child turning 18 years old.
