Since May 2025, the Ministry of Health has introduced a new, free preventive program called “Moje Zdrowie” (“My Health”), which replaced the previous program “Profilaktyka 40 PLUS.” The program aims to encourage Poles to undergo regular preventive check-ups and to detect lifestyle and cancer-related diseases early.
What is the “Moje Zdrowie” program?
This is a comprehensive preventive program aimed at people aged 20 and above. Participants go through three main stages:
- Completing a health questionnaire: The questionnaire includes questions about lifestyle, family medical history, and risk factors for HCV infection.
- Undergoing recommended diagnostic tests: Based on the questionnaire results, the patient receives a referral for appropriate preventive tests.
- Receiving an Individual Health Plan (IPZ): The plan contains recommendations for further prevention and possible treatment.
The program is carried out through primary healthcare clinics (POZ) and aims at early detection of diseases.
Who can participate in the “Moje Zdrowie” program?
The program is available to anyone aged 20 or older. However, the frequency of participation depends on age: people aged 20–49 can participate every 5 years, while those aged 50 and above every 3 years. It is also worth noting that individuals who participated in the “Profilaktyka 40 PLUS” program can join “Moje Zdrowie” one year after their last tests.
How to participate in the “Moje Zdrowie” program?
- Internet Patient Account (IKP): Log in to your account and complete the online questionnaire.

- Primary healthcare clinic (POZ): Contact our clinic, where the medical staff will assist you in completing the questionnaire and conducting the necessary tests.
After completing the questionnaire, the system will automatically generate a referral for the appropriate preventive tests. The test results and the Individual Health Plan will then be available on your IKP account or in the mojeIKP app.
